Your shoulder may ache before you get dressed, then pinch when you reach for a shirt. Even a coffee cup can feel heavier in that hand. The smooth covering on the ends of the shoulder bones can wear down. The joint may also grow stiff. A tendon connects a muscle to a bone, and it can become sore or tear. You can’t reliably sort these causes by where the ache sits.

An exam matters when soreness has no clear cause.

Daily use can keep a sore shoulder stirred up

The shoulder moves in many directions, so small jobs can keep bothering it. Reaching overhead, pulling a door, golfing, or sleeping on one side may add strain. Leaving the arm still for days can stiffen the joint and weaken nearby muscles. Ease the hardest motion for now, but keep the arm moving where it feels comfortable. If the ache is stronger the following morning, shorten the work or use less force.

Calm, regular use is usually kinder than one hard effort.

The letters PRP mean platelet-rich plasma. Clinic staff take blood from your arm. A spinning machine separates a part holding more platelets, which are small pieces in blood that stop bleeding and send the body’s early signals after nearby tissue is hurt. Staff place that liquid in the sore shoulder. The aim is relief without surgery, but the visit can’t promise how much you’ll feel.

Sudden changes need faster care

Don’t wait for a regular visit after a fall leaves you unable to lift. A shoulder that looks bent or out of place also needs prompt help. Fever with a hot, red, swollen joint may mean an infection. New numbness, loss of hand control, or quickly growing weakness also needs medical care. These warning signs come before any planned shoulder treatment.

The Cochrane review of exercise for knee osteoarthritis found high-quality evidence that land-based therapeutic exercise provides short-term benefit in pain and physical function, sustained for at least 2-6 months after the programme ends, with mild transient soreness the only reported adverse effect across 45 trials. It is the best-evidenced treatment for this condition and it costs nothing per injection.

  3. The RESTORE trial - a participant-, injector- and assessor-blinded RCT of 288 adults aged 50+ with symptomatic medial knee OA (Kellgren-Lawrence 2-3) - compared three weekly intra-articular PRP injections against saline placebo, with co-primary endpoints of 12-month knee pain and medial tibial cartilage volume on MRI. PRP did not beat placebo on either. It is the single best-designed test of the specific claim that PRP changes joint structure, and it was negative.

In the MOON Shoulder prospective cohort of 452 patients with symptomatic, ATRAUMATIC full-thickness rotator cuff tears, physical therapy succeeded in more than 70% of patients at 10 years: only 115 (27.0%) had surgery at any point over the decade. Patient-reported outcomes improved with physical therapy and did NOT decline over 10 years in those who never had surgery. The strongest predictor of early surgery was low patient expectation of physical therapy, not tear anatomy.

The GRASP trial randomized 708 adults with a rotator cuff disorder to progressive exercise (up to 6 sessions), a single best-practice advice session, or either of those preceded by a corticosteroid injection. Over 12 months there was no evidence of a difference in Shoulder Pain and Disability Index between progressive exercise and one advice session (adjusted mean difference -0.66, 99% CI -4.52 to 3.20), and no evidence of a difference between having a corticosteroid injection and not having one.

    Hopewell S, et al. — Progressive exercise compared with best practice advice, with or without corticosteroid injection, for the treatment of patients with rotator cuff disorders (GRASP): a multicentre, pragmatic, 2 × 2 factorial, randomised controlled trial..   6. A 2026 meta-analysis of 10 randomized trials (n=591) found PRP and corticosteroid indistinguishable at 3-6 weeks and 3 months, with PRP pulling ahead at 6 months: ASES +10.8 (95% CI 4.71-16.80), Constant-Murley +10.7 (1.21-20.27) and VAS pain -0.8 (-1.45 to -0.18), plus fewer adverse events (RR 0.66, 0.44-0.99). The authors describe the benefit as statistically significant but CLINICALLY MODEST.
