Some shoulder soreness can wait, but sudden weakness should not
A familiar morning ache may ease after you move around. That slower soreness often gives you time to lighten the work and arrange an exam. A shoulder that changes suddenly after a fall is different. New weakness, an arm you can’t raise, or a joint that looks bent needs prompt care. Fever or feeling ill makes the need more urgent.
Sudden change calls for faster help.
A slow ache often allows a measured start
Soreness that grew over months may come from joint wear, stiffness, or a sore cord joining muscle to bone. Comfortable movement and lighter daily work may help while you wait for an exam. A scan can show an old tear that caused no trouble before. Your strength, motion, sleep, and the day the ache began help the doctor judge the scan. If these are steadily improving, surgery may not be needed.

This kind of appointment is not meant for a broken bone, car crash, hard fall, or sudden loss of arm strength. The shoulder exam happens during the visit and helps narrow the cause. It can’t give every person a certain answer.

Heat, swelling and sudden weakness need prompt attention
Fever beside a warm, red, puffy shoulder may mean an infection. Seek care before a planned visit when those signs are present. If the arm hangs oddly or won’t rise after a fall, get help quickly. New numbness, loss of hand control, or growing weakness also needs quick medical care. Unexplained weight loss with worsening night pain deserves an exam soon.

In the MOON Shoulder prospective cohort of 452 patients with symptomatic, ATRAUMATIC full-thickness rotator cuff tears, physical therapy succeeded in more than 70% of patients at 10 years: only 115 (27.0%) had surgery at any point over the decade. Patient-reported outcomes improved with physical therapy and did NOT decline over 10 years in those who never had surgery. The strongest predictor of early surgery was low patient expectation of physical therapy, not tear anatomy.

In a nationally representative Finnish population sample of 602 adults aged 41-76 who had bilateral 3-Tesla shoulder MRI regardless of symptoms, rotator cuff abnormalities were found in 98.7% of participants (25% tendinopathy, 62% partial-thickness tear, 11% full-thickness tear). Abnormalities were present in 96% of ASYMPTOMATIC shoulders. Only full-thickness tears were more common in symptomatic shoulders, and that difference all but disappeared after adjustment (absolute difference 0.8%, 95% CI -3.4% to 6.0%).

The August 2025 AAOS Evidence-Based Clinical Practice Guideline on Management of Rotator Cuff Injuries issued 25 recommendations and 4 consensus statements. Among its updates it clearly RESTRICTS the use of platelet-rich plasma and marrow stimulation in rotator cuff repair, limits prolotherapy in full-thickness tears, establishes CT as an adjunctive imaging modality, and endorses early mobilization after repair of small-to-medium tears.

FDA states plainly that no stem cell, exosome, stromal vascular fraction, umbilical cord blood, Wharton's jelly or amniotic-fluid product has been approved for the treatment of ANY orthopedic condition - it names osteoarthritis, tendonitis, disc disease, tennis elbow, back pain, hip pain, knee pain, neck pain and shoulder pain individually. The only FDA-approved stem cell products in the United States are cord-blood-derived blood-forming stem cells for disorders of the hematopoietic system, and there are currently no FDA-approved exosome products.

A four-arm, multicentre, single-blind phase 2/3 randomized trial of 480 knee OA patients (KL II-IV) compared autologous bone marrow aspirate concentrate, autologous adipose stromal vascular fraction and allogeneic umbilical-cord-tissue mesenchymal stromal cells against a corticosteroid injection control. At 12 months NONE of the three orthobiologic injections was superior to another, or to the corticosteroid control, and none of the four groups showed a significant change in MRI osteoarthritis score from baseline. No procedure-related serious adverse events occurred.

A 2026 systematic review and meta-analysis of 28 randomized trials of intra-articular mesenchymal stem cell-based therapies in knee OA found significant improvements in several pain and function measures (delta-VAS MD -1.67; KOOS pain MD 15.37) but NO significant difference in WOMAC, KOOS quality of life or the Lequesne index, and MRI-based WORMS scores were non-significant - indicating no consistent structural benefit. Its own conclusion: these therapies serve a primarily SYMPTOM-modifying rather than STRUCTURE-modifying role, with higher frequencies of local reactions to weigh against the symptomatic benefit.
